Autism Spectrum Disorder (ASD) is a neurodevelopmental condition characterized by differences in social communication, interaction patterns, and the presence of restricted or repetitive behaviors. The spectrum ranges from individuals requiring minimal support to those needing substantial daily assistance. Many individuals with ASD experience significant co-occurring conditions including anxiety, sensory sensitivities, sleep disturbances, seizures (approximately 20–30% of ASD individuals), self-injurious behaviors, and emotional dysregulation — and it is often these co-occurring symptoms that most impact quality of life.

A landmark Israeli study (Aran et al., Journal of Autism and Developmental Disorders, 2019) assessed CBD-rich cannabis (20:1 CBD:THC ratio) in 60 children with ASD and severe behavioral problems. Behavioral outbreaks were "much improved" or "very much improved" in 61% of patients, anxiety improved in 39%, and communication improved in 47%. A follow-up randomized, placebo-controlled trial (Aran et al., Molecular Autism, 2021) — the first RCT of cannabinoids in autism — enrolled 150 participants. On the Clinical Global Impression-Improvement scale, approximately 50% of children in both cannabinoid treatment arms improved compared to only 20% on placebo, representing a clinically meaningful difference. A separate observational study found that 90% of ASD participants showed improvement in at least one core symptom with CBD-enriched cannabis. CBD-rich formulations with minimal THC (20:1 ratio) are the most studied and appear safest for ASD. The anxiolytic properties of CBD are particularly relevant — anxiety is a major driver of behavioral outbursts in ASD, and reducing anxiety often improves social engagement and decreases avoidance behaviors. For the 20–30% of ASD individuals who also have epilepsy, CBD's well-established anticonvulsant properties provide additional benefit.
